The shoulder joint is the most flexible joint in the human body allowing us to engage in a variety of activities such as swimming, playing badminton, Tennis and gymnastics. However increased mobility means decreased joint stability and makes the shoulder joint particular susceptible to dislocation we have various ways of reducing the risk of dislocation of the shoulder joint one of which is through the muscles that we are going to study, the Muscles of the rotator cuff.

First we will look into the bones that make the shoulder joint as they form the bony frame work and then we will learn about the Muscles of the rotator cuff.

Bones of the shoulder joint

As we know shoulder joint is the most mobile joint in the human body and it is due to the fact that itś  a synovial ball and socket joint. The ball component of this joint is formed by the Humerus which is a feature of the long bone of our arm  the humerus whereas the socket component is formed by the glenoid cavity which is a part of the scapula or the shoulder blade and as such the shoulder joint is also known as the Glenohumeral joint.

SITS Muscles

The shoulder is supported by four rotator cuff muscles also called the SITS muscles.

  • Superspinatus
  • Infraspinatus
  • Teres Minor
  • Subscapularis

The SITS muscles form a cuff that surrounds the head of the Humerus to make it stable and help it move, specifically when we are rotating hands. There is a rubbery substance filled near the infraspinatus known as synovial fluid, it provides lubrication and prevents the rotator cuff especially the superspinatus tendons against rubbing.

Rotator cuff tear may occur when the tendons are violently stretched like when you lift something which is too heavy or accidentally fall on an outstretched arm this most commonly affects the superspinatus tendon at its insertion on the greater tubercle of the head of the humerus, because its under lot of tension when the shoulder is abducted, now the tears may be partial which is where the tendon is damaged but not completely torn and even may go unnoticed. Alternatively the tear may be a full thickness tear where the tendon is completely detached from the humerus.

In contrast to an acute tear there may also be a chronic tear due to regular wear and tear which usually occurs in the elderly, that is because of the blood supply to the rotator cuff tendons reduces overtime making it harder for them to heal after an injury as a result injuries can go unprepared.

A classic example is to repeat the motion over and over like playing regularly doing activities such as badminton, swimming or playing tennis. Chronic tears can also occur where the bones overgrow specifically where there may be  a bone spur coming off of the acromian that rubs the rotator cuff tendons during shoulder abduction, this condition is called shoulder impingement.

In an acute tear there can be an intense shoulder or upper arm pain, whereas in chronic tear the pain might be absent or only occur when using the affected shoulder. The affected shoulder may also be weak specifically with abduction or rotation.

The treatment for rotator cuff begins with ice to reduce inflammation, pain medication and rest with gentle physical exercise to prevent stiffening. In more severe cases surgery may be needed to repair the torn tendon. But, before you opt for a surgery you might discuss the details with your doctor whether it can be cured by doing some exercises.

Start with weights at side, bring right arm to left shoulder bending elbow, palm up going up, palm down coming down

Exercise 3

Lay down and use the dumbbell to move your hand up and down slowly.
